In order to save and/or print the form WITH THE INFORMATION YOU ENTER, you must use the free Adobe Reader to enter and save the form with your information.
The free Adobe Reader is available from the Adobe website (You should de-select any "Optional Offers" before installing the software).
Sending by email
If you elect to send your completed application form(s) by email to AMI (Canada), please read this note about completing and emailing your filled-in form.
- Download a fillable 2022 Individual Membership Form
- Complete the form with your information, resave it, then send to membership@ami-canada.com
- For discounted, group memberships for 5+ or 10+ members from one school, download both the 2022 School Group Membership Form
Sending by mail
Mail completed Membership Form(s) along with payment to:

Upon receipt of your completed membership form we will issue you a password which will give you access to the Member's page. A new password will be issued each membership year.
Payment
- Payment may be made by cheque or money order payable to AMI (Canada) and mailed to the address above with completed membership form(s).
- Payment may also be made online by e-transfer (info@ami-canada.com) .
